Join us in our fight against childhood
obesity!
The Cedarburg Physical Therapy Foundation needs your help as we
host our
fourth annual charity golf outing. We are a not for profit 501(c)3
corporation and have dedicated ourselves to raising money for the
health and
wellness of children. We are seeking golfers and sponsors to help
us raise
money for this very worthy cause.
In 2007, we were able
to donate $3500 to the Cedarburg Education
Foundation (CEF) earmarked for health, wellness and physical education
programs in our schools. Through our donation, we were able to
purchase
three Dance, Dance Revolution® interactive gaming systems for
our
elementary schools. These systems will allow our physical education
teachers to integrate a high tech work out into their curriculum
for benefit of
our children.
We welcome your sponsorship
or gift donation, but mostly we would like
you to participate in our golf outing. It promises to be a fun
filled day of
golf for a great cause. We hope that you will consider joining
us for this
annual tradition as we welcome golfers of all abilities. If you
are unable to
join us for golf, please consider making a donation to our Foundation.

Everyone hits a tee shot, then each person plays from the area of the
best shot and must be within 1 foot, no closer to the hole in the same type of playing
surface. (best shot in the rough means each person plays from the rough, etc.) |

match you with others to make a competitive and fun team. |

We ask that you consider
a donation of any size to help support our charity golf outing. There
are several different levels of sponsorship available. All
donations are 100% tax deductible through the IRS. All sponsors will
be acknowledged on the day of the event. Donations for prizes are
also greatly appreciated and can be dropped off at Cedarburg Physical
Therapy any time prior to the outing.We hope to receive all donations
by Friday, June 20 to allow us enough time to organize prizes and
the silent auction. |
